The purpose of this letter is to request a DD-214 which shows all awards that I am entitled to which are directly related to the following operations that I participated in:

List All Operations you were involved in:

a.) Cambodia Evacuation Eagle Pull (April 11 to 13, 1975).
b.) Vietnam Evacuation Frequent Wind (April 29 to 30, 1975).
c.) Mayaguez Operation (May 15, 1975).

2. I'm submitting the following additional information so that you can verify my participation in these important Vietnam era combat operations.
(Note) Send a copy of your original DD-214 (if you have it). If you don't have your original DD-214, send this letter along with your SSN information should be enough.
